Big Bend Community Based Care

Big Bend Community Based Care (BBCBC), as the lead agency for 14 counties in the Big Bend region of Florida, provides administrative and hands-on Child Abuse Prevention, Case Management, Foster Care Placement, Independent Living, and Adoption Services to its partner agencies and clients. BBCBC serves as the central location from which partner agencies can draw resources and support, and offers hands-on support to foster and adoptive parents, volunteers and case managers.

BBCBC engaged Five Points in 2006 to build the Purchase of Services (POS) System. This utilization management system is designed to track expenses for clients in care. Five Points worked with BBCBC management and users to design, develop, test and implement a service tracking system based on Five Points’ Community Based Resource Information System (CoBRIS). Five Points staff documented and reengineered the service request process while implementing a paperless system for service referral and tracking.

Five Points conducted process analysis, gathered and defined user requirements, developed the application, conducted user training, and currently hosts the system from its remote secure data processing center with disaster recovery and business continuation plans in place. Five Points provides ongoing maintenance and enhancement development services for the POS system.
